The Cigna Group reported stronger third-quarter operating performance on Thursday, November 5, 2026, and raised its full-year earnings outlook. The company said in its company announcement published through Financial Times that total revenue increased 6 percent year over year to 73.7 billion US dollars for the quarter ended September 30.

Adjusted income from operations reached 2.1 billion US dollars, or 7.98 US dollars per diluted share, compared with 7.83 US dollars a year earlier. Shareholders' net income fell to 1.4 billion US dollars, or 5.47 US dollars per share, from 1.9 billion US dollars, or 6.98 US dollars, in the prior-year quarter. The company attributed the decline mainly to the absence of an after-tax gain on a sale recorded in the third quarter of 2025.

Insurance operations drive earnings momentum

Cigna Healthcare delivered the clearest operating improvement. Adjusted revenue increased 9 percent to 11.7 billion US dollars, while pre-tax adjusted income from operations rose 15 percent to 1.2 billion US dollars. Cigna said the increase reflected higher net investment income and improved operating efficiency. Its medical care ratio was 85.0 percent, compared with 84.8 percent a year earlier, with business mix affecting the comparison.

Evernorth Health Services produced adjusted revenue growth of 6 percent, but pre-tax adjusted income from operations declined 4 percent to 1.8 billion US dollars. Pharmacy Benefit Services revenue rose 9 percent, primarily on drug mix, while its operating income declined 22 percent amid large client renewals and strategic investments. Specialty and Care Services posted a 15 percent increase in pre-tax adjusted income from operations to 1.1 billion US dollars.

Higher floor sets 2026 benchmark

The company raised its 2026 consolidated adjusted income from operations outlook by 0.15 US dollars per share to at least 30.60 US dollars. The forecast includes expected future share repurchases and anticipated 2026 dividends. Cigna Healthcare's full-year pre-tax adjusted operating income target increased by 50 million US dollars to at least 4.6 billion US dollars, while the medical care ratio outlook narrowed to 84.2 percent plus or minus 25 basis points.

Chief Executive Officer Brian Evanko said the group is focusing on core growth, differentiated capabilities and disciplined execution. He also linked the results to the multi-year Accelerate to One modernization and productivity initiative, which the company introduced alongside longer-term targets at its 2026 Investor Day.
