HCA balances growth and payer pressure

  • Third-quarter revenue rose 3.6 percent, while attributable net income fell 3.9 percent.
  • Management expects 2027 earnings to remain generally consistent with 2026 as exchange coverage losses and lower Medicaid payments weigh on growth.
  • Full 2027 guidance is due with the fourth-quarter report in January.

HCA Healthcare reported stronger demand but continued payer pressure in its third quarter ended September 30, 2026. The company said on October 27 that revenue rose 3.6 percent to 19.849 billion US dollars, while net income attributable to HCA Healthcare fell 3.9 percent to 1.579 billion US dollars.

The quarterly release from HCA Healthcare showed adjusted EBITDA of 3.918 billion US dollars, up 1.2 percent from a year earlier. Same-facility equivalent admissions increased 3.0 percent, and emergency-room visits rose 5.1 percent. The operating momentum was offset by a shift toward uninsured patients after some former exchange customers lost coverage.

Payer mix remains the central risk

HCA estimated that the exchange-to-uninsured shift reduced income before income taxes by approximately 400 million US dollars in the quarter. Management said the effect was driven primarily by patients who lost health-insurance exchange coverage. Exchange equivalent admissions declined 19 percent year over year but improved 8 percent from the second quarter.

Uninsured patients represented 10.5 percent of equivalent admissions in the quarter, compared with 8.5 percent a year earlier. The pressure was visible in elective care: same-facility inpatient surgeries declined 1.4 percent and outpatient surgeries fell 3.7 percent from the prior-year quarter.

HCA nevertheless raised its confidence in the current year. The company expects 2026 operating results to land in the upper half of the previously issued guidance ranges, which call for revenue of 77.0 billion to 79.5 billion US dollars and adjusted EBITDA of 15.4 billion to 16.1 billion US dollars.

Early 2027 view points to flat earnings

On the October 27 earnings call, Chief Executive Officer Sam Hazen said demand should support volume growth of 2 percent to 3 percent in 2027. He also said fewer people are expected to lose exchange coverage than in 2026, although the payer-mix effect should remain meaningful. HCA plans to give more detailed 2027 guidance after it has seen fourth-quarter performance and the results of the open-enrollment period that began November 1.

Management also expects Medicaid supplemental-payment net benefits to decline by 550 million to 650 million US dollars in 2027. Against those pressures, HCA is relying on volume growth, cost initiatives and technology investments. The preliminary conclusion is that 2027 earnings will be generally consistent with 2026, with a return to longer-term growth dependent on a better payer mix and stable policy conditions.
