When the Da Vinci XI surgical system arrived at Tuttlingen’s district hospital in March 2026, the €1.5 million investment was a bet on precision and shorter patient stays. Eight months later, surgeons have performed over 100 procedures with the robot—mostly in visceral surgery and gynaecology—and the double console design is already being used to train the next generation of specialists.

Yet the hospital’s leap into robotic surgery stands out precisely because it remains rare in Germany. According to industry data, the country’s automation density sits at 440 robots per 10,000 employees, or roughly 4.4 percent. South Korea, by contrast, has 12 percent—nearly three times the rate. While China’s robotics market has grown 15 percent annually over the past five years, Germany has managed just 4 percent growth. Experts see low automation as a missed opportunity to relieve pressure on an overstretched healthcare workforce.

The gap is especially stark in surgery, where robot-assisted tools can cut recovery times and reduce physical strain on doctors. At Tuttlingen, the system’s ergonomic console means surgeons no longer have to stand for hours in awkward positions. The shorter in-patient stays that follow also free up bed capacity. Both factors help address the chronic staffing shortages that plague German hospitals.

Elsewhere, robot-assisted orthopaedics is gaining ground. The Helios Klinikum Meiningen, for instance, now uses a CT-based system to plan knee replacements with millimetre precision. Meanwhile, researchers at Dresden University Hospital and TU Dresden are developing AI assistants for complex abdominal cancer surgery. The project lead, Fiona Kolbinger, received the Jung Career Promotion Award in 2026 for her work on the technology.

On the business side, endoscopy specialist EMOS Technology is moving its headquarters from Illmensee to Pfullendorf, where it will build a 4,500-square-metre facility for administration and production. The relocation follows a 94 percent revenue jump between 2022 and 2025. The company, which currently employs 65 people, plans to hire more than 100. The entire group already has around 270 staff on its payroll. Management described the expansion as a strategic necessity for future growth.

In Schwäbisch Gmünd, robot maker Estun opened a new location in June 2026, adding to the patchwork of automation projects scattered across Germany. But the overall pace remains slow. One reason may be the low starting salaries for specialists: a fully trained physician begins at around €5,073 a month, while a chief surgeon can earn up to €16,000. For many hospitals, a seven-figure robot investment is a multi-year decision.

Workplace mental health is another area where regulation has moved ahead of adoption. Since 2013, German employers have been required to include psychological strain in their mandatory risk assessments. The Federal Institute for Occupational Safety and Health (BAuA) enforces the rule under standard DIN EN ISO 10075-1, part of the Joint German Occupational Safety and Health Strategy (GDA). The GDA’s targets include giving workers enough decision-making latitude, controlling workload volume, ensuring social support, and making working hours predictable. A 2025 BAuA publication offered concrete design options for mental health at work.

Despite these requirements, the automation gap persists. Germany’s robot density of 440 per 10,000 employees remains far below the level experts say is needed to compensate for demographic decline and rising demand for care. Robotics, proponents argue, is not just a tool for efficiency—it is a lever to keep healthcare workers healthy enough to stay on the job.
