The market reaction to the J&J news was swift, but it landed on a stock that was already in distress. Intuitive shares had fallen roughly 35 percent from the start of the year before the FDA announcement, and the past 30 days alone had seen a decline of 15.81 percent. The trigger for that selloff was the company’s second-quarter earnings release on July 16, which delivered strong operational numbers but a stubbornly conservative outlook.

While executives described the likely impact as “moderate,” the decision to hold the guidance steady signaled caution about the second half of the year.

The convergence of regulatory and earnings headwinds has left Intuitive shares trading at €298.65, just 0.20 percent above the 52-week low of €298.05 set on July 23. The 14-day relative strength index has fallen to 30.9, deep in oversold territory, reflecting the intensity of the selloff that began well before the J&J news broke.
Options activity tells a similar story. The put-call ratio for contracts expiring at the end of August stands at 1.29, a distinctly bearish signal. The lowest strike price among those contracts is $317, implying traders are positioning for a potential further decline of roughly 7 percent from current levels.
A Moat Under Pressure
Despite the mounting challenges, Intuitive retains a commanding market position. The company held a 57.6 percent share of the U.S. robotic surgery market in 2025, with Stryker and J&J’s own Auris Health trailing in second and third place. The overall market continues to expand, with projections calling for the global robotic surgery market to grow from roughly $12 billion to $20.8 billion by 2035, a compound annual growth rate of 5.6 percent.

The near-term threat from Ottava is less about an immediate loss of market share — J&J is starting with a limited number of customers — and more about the erosion of Intuitive’s pricing power. That dynamic makes it harder to justify the premium valuation the stock has historically commanded.
A Fortress Balance Sheet
What cushions the blow is Intuitive’s financial strength. The company ended the second quarter with $8.63 billion in cash and investments, and free cash flow for the first half reached $1.8 billion, up 71 percent from the same period last year. That liquidity has funded $379 million in share buybacks during the first six months of 2026, along with continued heavy investment in research and development for endoluminal and digital platforms.
The balance sheet provides a buffer while the market digests the twin uncertainties of a new competitor and a potentially softer demand environment. How quickly J&J scales its Ottava installations will shape the competitive landscape in coming quarters, as will the actual impact of the ACA subsidy expiration on procedure volumes. For now, Intuitive’s management describes that impact as moderate — but the market is pricing in a scenario that is considerably more cautious.
