Intuitive Surgical delivered a second-quarter earnings beat that would typically send shares higher. Instead, the stock has been hammered, with investors fixated on a deceleration in procedure growth that threatens the premium valuation the company has long commanded.

The robotics surgery leader earned $2.80 per share on a non-GAAP basis in Q2 2026, comfortably ahead of the $2.51 consensus estimate, while net income hit $1 billion. Yet the market’s attention pivoted instantly to the outlook: management guided for full-year da Vinci procedure growth of 13.5% to 15.5%, with U.S. expansion slowing from 14% in the first quarter to 12% currently.

That cooling has been enough to trigger a reassessment. The stock has shed 39.42% since the start of the year, closing Friday at €296.35 — a 1.58% gain that barely dented the damage from Thursday’s fresh 52-week low of €289.05. The shares now trade 16% below their 50-day moving average and nearly 30% below the 200-day line, with the gap to the 52-week high of €516.50 standing at 42.62%.

A Shift in How Hospitals Buy

The operating picture, however, shows no fundamental weakness. Intuitive Surgical placed 468 da Vinci systems in the second quarter, including 246 of the next-generation da Vinci 5 — up from 395 total placements a year earlier. What has changed is the financing model: 54% of system placements now go through operating leases, versus 49% a year ago. Hospitals constrained by capital budgets are lowering their upfront costs, which reduces immediate revenue for Intuitive but locks in recurring income from service contracts and instruments over the lease term.

Should investors sell immediately? Or is it worth buying Intuitive Surgical?

An additional headwind comes from the demand side. Management pointed to the expiration of insurance subsidies in the U.S., which is discouraging patients from scheduling elective procedures — a factor that directly weighs on the procedure growth rate that investors are scrutinizing.

The AI Bet as a Competitive Moat

While the market frets over near-term growth, the company is laying groundwork for a longer-term advantage. At the Society of Robotic Surgery annual conference in Hollywood, Florida, CEO Dave Rosa and SVP Iman Jeddi unveiled an “AI Capability Stack” built on a global dataset of more than 20 million da Vinci procedures.

The framework spans five layers: clean, standardized surgical data at scale; clinically actionable insights from that data; real-time intraoperative support; AI-enhanced precision for surgeon hand movements; and what the company calls “supervised autonomy” — systems that expand the range of action without ceding human control.

The strategic logic is straightforward: Intuitive Surgical wants to transform itself from a hardware vendor into a data platform that competitors will find difficult to replicate. The da Vinci 5 rollout, with more than half of new placements now going to the latest model, is the delivery mechanism for that platform.

Institutional Interest and Competitive Pressure

The stock’s decline has begun attracting buyers looking for value. Unisphere Establishment disclosed in a Friday SEC filing that it had built a new position of 4,600 shares. The entry point coincides with fresh competitive pressure: Johnson & Johnson’s Ottava system received De Novo clearance from the FDA this week, opening a new front in the battle for operating room dominance.

Intuitive Surgical at a turning point? This analysis reveals what investors need to know now.

Analyst reactions to the quarterly numbers have been mixed but not uniformly negative. BTIG and Baird trimmed their price targets, citing softer U.S. procedure growth tied to changing insurance conditions and a drop in bariatric procedures. Most firms, however, maintained positive ratings, pointing to the da Vinci 5 upgrade cycle as a defensive moat against new entrants.

Technical Signals Point Both Ways

The 14-day relative strength index sits at 31.9, just above the threshold of 30 that chartists typically associate with oversold conditions — a setup that has historically preceded technical bounces. Friday’s close above €296.35 puts the €300 level in focus for the coming week. A sustained hold above that mark could signal the first step toward stabilization; a break back below the July 23 low would suggest the downtrend has further to run.

The broader question hanging over the stock is whether the U.S. procedure slowdown is a temporary blip or the start of a more lasting deceleration. The answer will determine whether the current repricing is a buying opportunity or a new equilibrium.
