For the three months ended June 2026, Intuitive Surgical generated revenue of $2.89 billion, up 19% from a year earlier. Adjusted earnings per share came in at $2.80, well ahead of the consensus estimate of $2.48. The company also posted a 30-basis-point improvement in adjusted gross margin to 70%, while adjusted operating margin expanded by 330 basis points to 42%.

The system is the first of its kind approved in the U.S. and reportedly reduces operating room footprint by 30% to 50%. Initial cleared indications include gastric bypass, gastrectomy, cholecystectomy, sleeve gastrectomy, appendectomy, and hiatal hernia repair.

Johnson & Johnson plans a phased commercial rollout, with expansion into Japan and Western Europe to follow. The entry directly challenges da Vinci and Medtronic’s Hugo system in a market where only about 8% of surgeries globally are currently robot-assisted — leaving plenty of room for competition.

A J.P. Morgan analyst noted that Johnson & Johnson will likely pursue additional approvals for urological procedures, a segment where Intuitive Surgical remains dominant with da Vinci.

Valuation Compression and Analyst Divergence

The stock now trades at 9.89 times expected annual revenue, its lowest multiple in 18 months and well below the 15.89 median. The 14-day relative strength index stands at 31.3, signaling oversold conditions. Over seven trading days, the shares lost nearly 15% of their value.

Analyst reactions have been mixed but largely cautious. Bernstein cut its price target from $750 to $685, while Stifel slashed its target from $670 to $550. Citigroup lowered its target to $500, and TD Cowen reduced its to $520. Goldman Sachs held firm at $558. Deutsche Bank stands out with a sell rating and a $366 target. Simply Wall St trimmed its fair-value estimate from $565.25 to $501.73, citing a lower revenue growth assumption of 12.14% versus the prior 13.09%.

Dimensional Fund Advisors increased its stake by 3.6% in the first quarter to 645,181 shares, while CI Investments cut its position by 33.1%, selling 61,937 shares. Insider sales added to the negative signal: SVP Iman Jeddi sold 5,625 shares in early June at $420.55, and EVP Gary Loeb disposed of 400 shares in mid-June at $424.14.

Balance Sheet Provides a Floor

Despite the stock’s slide, Intuitive Surgical’s financial position remains robust. The company ended the quarter with $8.63 billion in cash and investments. Free cash flow for the first half reached $1.8 billion, up 71% from the prior-year period. That liquidity supports $379 million in share buybacks during the first half and continued investment in R&D for endoluminal and digital platforms.
