The company placed 468 new da Vinci systems and 55 ION bronchoscopy systems during the period, while the non-GAAP operating margin clocked in at 42%. The ION platform saw procedure volume surge 36%, and overall da Vinci procedure growth hit 16%. Yet the forward outlook soured the mood.

Management reaffirmed its full-year procedure growth guidance of 13.5% to 15.5%, a range that disappointed a market hoping for an upgrade after such a strong quarter. The problem lies in the U.S., where da Vinci procedure growth slowed to roughly 12% — the weakest pace since the pandemic years of 2020 and 2021. The installed base in the U.S. expanded just 8.7%, and executives attributed the deceleration to a combination of insurance disenrollments tied to the Affordable Care Act and the simple math of a larger base, where sustaining double-digit percentage growth becomes increasingly difficult. Adding to the concern, hospital operator HCA recently reported declining surgical volumes, feeding doubts about the broader U.S. procedure environment. China was also flagged as a challenging market.

TD Cowen dropped to $520 and maintained "Buy." Bernstein, among the most bullish, trimmed from $750 to $685 while keeping "Outperform." On the bearish side, Deutsche Bank downgraded the stock to "Sell" with a $366 target, and Leerink cut to $454. Citi held at "Buy" but lowered to $500, stripping its near-term catalyst call. The consensus price target fell from roughly $570 to about $504, with the stock now trading well below even the reduced average. The First Group Bank trimmed its fiscal 2027 EPS estimate to $9.91 from $9.94.

Institutional investors had already been trimming exposure before the earnings hit. Mediolanum International Funds reduced its stake by 5.9% in the first quarter, and Jennison Associates sold 5.2% of its holding, or 168,127 shares, while still retaining more than 3 million shares valued at around $1.4 billion. Insiders also sold $3.06 million worth of equity over the past 90 days.

Adding to the regulatory overhang, a Class II recall affecting 454 da Vinci units remains in effect. The recall, initiated in April 2026, involves a faulty retention pin on the emergency brake that could compromise braking or cause uncontrolled movement of the system column. While the recall itself is not a new event and was not the trigger for the current sell-off, it keeps the Food and Drug Administration's attention fixed on the company precisely when investors are already questioning the core growth story.

Technicians point to a support zone around $344 and a resistance band between $380 and $390 in U.S. dollar terms, levels that could define the next directional move. The stock now sits nearly 28% below its 200-day moving average, a stark measure of how far it has diverged from its longer-term trend. Whether the oversold condition sparks a bounce or gives way to further selling will hinge on how the market interprets the U.S. slowdown — as a temporary, policy-driven headwind or the beginning of a structural deceleration in the company's core business.
