Novo Nordisk's Strategic Gamble: Price Cuts and New Drugs in a Shifting Market

Novo Nordisk is deploying a multi-pronged strategy to defend its market position, combining aggressive pricing for a new weight-loss drug with a landmark FDA approval, even as patent expiries in key emerging markets squeeze revenue. The Danish pharmaceutical giant faces a pivotal year, with its share price down roughly 28% since January, hovering just above its 52-week low of EUR 30.48.
The company's commercial offensive centers on the U.S. launch of Wegovy HD, a new 7.2-milligram high-dose version of its blockbuster weight-loss injection. Priced at $399 per month for self-payers, it undercuts the comparable highest dose of rival Eli Lilly's Zepbound by $50. This direct price attack is a significant shift in strategy, aimed at recapturing market share in a space where Lilly now controls over 60% of the U.S. injectable market. Novo Nordisk is supplementing this move with new annual subscription plans that could save patients up to $1,200 per year.
This aggressive pricing is partly a response to recent clinical setbacks. In February, Novo Nordisk's combination therapy CagriSema failed to outperform Lilly's Tirzepatide in a head-to-head weight loss study. With an FDA decision on CagriSema not expected until late 2026, the company is relying on Wegovy HD to bridge the gap. The competition is also intensifying in oral medications; as Eli Lilly begins broad distribution of its new weight-loss pill Foundayo, Novo Nordisk is countering with simulated study data claiming advantages for its own Wegovy pill in weight reduction and lower discontinuation rates.
Beyond the weight-loss battle, the company scored a significant win in diabetes care. In late March, the FDA approved Awiqli, the first long-acting basal insulin for adults with type-2 diabetes requiring only a once-weekly injection. This marks the introduction of the first new basal insulin class in over two decades. Clinical trials involving approximately 2,680 patients showed it achieved a greater reduction in long-term blood sugar for new users than the previous standard therapy. A U.S. launch in the familiar FlexTouch pen is planned for the second half of 2026.
Should investors sell immediately? Or is it worth buying Novo Nordisk?
However, these advancements are countered by severe pressure in emerging markets. The core patent for semaglutid—the active ingredient behind Ozempic and Wegovy—expired in India on March 20. Within 48 hours, over 15 generic versions from local manufacturers flooded the market, causing prices to collapse by up to 90%. To remain competitive, Novo Nordisk was forced to slash prices for its own branded products by up to 48% and is now pursuing distribution partnerships with Indian firms like Emcure and Abbott India.
This pattern is set to repeat. The basic patent protection for semaglutid also ended in Canada, China, and India in the first quarter of 2026, opening the door for the first copycat products. To mitigate these losses, Novo Nordisk is accelerating the switch of patients in these regions to newer formulations like Wegovy HD and the Wegovy pill. In core markets like the U.S. and Europe, critical secondary patents on delivery systems and specific compositions remain in force until the early 2030s.
The fundamental challenges are reflected in the company's financial guidance. For 2026, management forecasts a currency-adjusted sales decline of 5% to 13%, citing U.S. government pricing agreements, reduced reimbursement for obesity medications, and Lilly's market dominance. To support its share price, Novo Nordisk is executing a share buyback program of up to DKK 15 billion, having already repurchased nearly 10 million shares by early April.
Novo Nordisk at a turning point? This analysis reveals what investors need to know now.
The next major catalyst for the stock is clear: the FDA's decision on CagriSema, expected around the turn of the year 2026/2027, remains the most critical strategic milestone ahead of the capital markets day in September 2026.
