BioNTech's Clinical Scorecard: One Endpoint Met, Several Questions Left Open

BioNTech shares came under pressure on Monday, dragged down by a sharp slide in Moderna. According to media reports, results from a Phase 3 trial of Moderna's melanoma vaccine weighed on the German biotech as well: the benefit in terms of distant metastases reportedly fell short of earlier expectations. It was a reminder that sentiment in the sector travels fast — and that BioNTech's own story is best judged on its own data.

A primary endpoint hit, with an asterisk

The already-presented Phase 3 DYNASTY-Breast02 trial of trastuzumab pamirtecan, developed with DualityBio, met its primary endpoint. In patients with metastatic, hormone-receptor-positive, HER2-low breast cancer, the drug improved progression-free survival versus physician's choice of chemotherapy by a statistically significant margin — but only a moderate one.

That distinction matters. Statistical significance and the size of a benefit are two different statements, and reading the headline alone leaves out a substantial part of the assessment. Overall survival data were not yet mature. The companies also reported a higher rate of interstitial lung disease than in earlier studies, including a small proportion of severe or fatal cases. Both firms said the potential benefit could be limited given the existing treatment landscape, and that they will discuss next steps.

None of this supports a blanket rejection of the drug — nor does it justify treating a met endpoint as a fully clarified risk-benefit profile.

Should investors sell immediately? Or is it worth buying BioNTech?

Combinations, and the discipline of reading them separately

A second thread at BioNTech is the testing of drug combinations, where the question is not only whether a single candidate shows activity but what each combination adds. The various studies should not be rolled together into a single verdict.

For pumitamig plus docetaxel in advanced non-small cell lung cancer after immuno- and chemotherapy, BioNTech and Bristol Myers Squibb reported a confirmed objective response rate of 36.7% across 30 efficacy-evaluable patients. Median progression-free survival in the full treated group of 50 patients was 6.9 months. The companies explicitly labeled the Phase 2 ROSETTA Lung-107 data as early, and the study had no comparator arm. An response rate and a progression-free survival figure do not describe the same thing; these are separate clinical findings, not mutual confirmation of every development program.

The collaboration with Gilead Sciences, agreed just over a week ago, extends the combination approach. Pumitamig is to be tested alongside sacituzumab govitecan in previously untreated, locally advanced or metastatic triple-negative breast cancer with low or absent PD-L1 expression. BioNTech will lead the trial and adjust the Phase 3 ROSETTA Breast-01 study accordingly; Gilead will supply the drug. The planned amendment remains subject to regulatory approval. Comparing the combination against sacituzumab govitecan alone would sharpen the focus on the incremental contribution — but a cooperation agreement does not answer that question. It creates the framework to investigate it clinically.

The next checkpoint

BioNTech will publish its third-quarter 2026 results and a corporate overview on November 3, with an investor call scheduled for 14:00 Central European Time. The clinical yardstick remains the decisive one: acknowledge demonstrated benefit, retain the limitations, and do not book planned studies as completed successes. BioNTech offers several development approaches; each must be measured against its own results.
