Intuitive Surgical Faces a Two-Pronged Test: Barclays Backs the Story as J&J and the FDA Close In

Fresh coverage from Barclays has handed Intuitive Surgical a vote of confidence, but the robot-surgery pioneer is simultaneously staring down a rival's commercial launch and a tightening regulatory net in Washington.
The British investment bank initiated coverage of the Sunnyvale, California-based medtech group with an "Overweight" rating. Its analysts expect the broader medical technology sector's fundamentals to steady in 2027, with investor sentiment brightening alongside them. That view lands against a market backdrop that has been anything but kind to the stock: since the start of the year, the shares have shed 27 percent. Yet the last 30 days have brought a visible counter-move, with the stock up 14 percent over that stretch.
The picture on European trading floors was muted in the latest session. Intuitive Surgical closed Tuesday at EUR 359.45, down 0.8 percent. During today's German session, the stock was changing hands at EUR 358.55, a modest 0.2 percent decline.
A Cash-Rich Operator With a Valuation Question Mark
The company's operating profile remains vigorous. Revenue reached USD 11.03 billion, underpinned by quarterly sales growth of 18.50 percent. That earning power shows up directly on the balance sheet: Intuitive Surgical carries no debt at all and holds USD 5.22 billion in cash. The liquidity cushion gives management room to maneuver while larger pharmaceutical and medtech players try to carve out their own positions in robotic surgery.
Should investors sell immediately? Or is it worth buying Intuitive Surgical?
Not everyone is sold on the price tag. Commentator Cramer pointed to the valuation gap relative to industry heavyweights such as Johnson & Johnson, whose free cash flow over the trailing twelve months hit USD 22.23 billion.
J&J's Ottava Looms Over the Operating Room
Competitive pressure on the pioneer is building as rivals bring their own platforms to market. Johnson & Johnson is preparing the commercial rollout of Ottava, a surgical robot whose design integrates four arms directly into the operating table. Media reports have circulated about an already-granted FDA clearance, though those accounts have so far gone without independent confirmation.
Regulators, meanwhile, are drafting stricter rules for the sector. For fiscal 2027, the FDA plans binding guidance on robotically assisted surgical systems. Interested parties have until November 24 to submit comments on the current draft.
Procedure Growth Anchors the Business
Clinically, Intuitive Surgical's momentum rests on steadily rising treatment volumes. In the second quarter of 2026, the global number of procedures performed with da Vinci systems climbed 16 percent year over year, while adjusted earnings per share for the same quarter came in at USD 2.80.
The generational shift in installed systems is a key pillar for future procedure counts. Of the 468 systems the company placed in the second quarter of fiscal 2026, 246 were the newer da Vinci 5 generation. Management has guided toward roughly 14.5 percent procedure growth for full-year 2026 — a figure that refers explicitly to the number of operations performed and should not be conflated with total revenue growth.
Intuitive Surgical at a turning point? This analysis reveals what investors need to know now.
Recurring Revenue, and a Price Cut on the Horizon
The economic foundation of the business leans heavily on recurring income. Against total revenue of USD 10.1 billion in fiscal 2025, instruments and accessories alone accounted for USD 6.0 billion. Robots sold to hospitals thus generate a continuous stream of follow-on orders for disposable and reusable components across their entire lifecycle.
For hospitals, though, pricing changes are coming. Starting in the first half of 2027, management plans to lower usage fees for selected instrument groups. The precise financial impact of those measures on margin development has not yet been quantified by the company.
